notice of privacy practices I

Located at the front desk of each of our health departments a copy of the notice of privacy practices and acknowledgement form in a white notebook. This book is available at anytime to the staff and public.

Each person entering the health department will receive a notice of privacy practices. They will in turn, if requesting services be asked to complete and sign the acknowledgement of receipt of privacy practices or the documentation of good faith efforts. A consent form will also be completed and signed by the client.

The acknowledgement will be filed in the clients file and noted on the computer of the date obtained.
